Your Ophthalmologist specifically referred you to our office
You are an immediate family member or in the same household as a current patient of our office.
You have been examined in the last decade by Dr. Lesli Handmacher or Kensington Optometry (Dr. Russell Cotteral & Assoc.)

New patients that qualify can fill out the form below and we will welcome you to the practice as availability permits. Please note that we are booking 4-8 weeks in advance.
